- Membership is billed monthly on the date of enrollment
- 3-month minimum commitment required
- Cancel anytime after 3 months with 30 days' written notice
- Child membership requires enrollment of at least one adult
- Annual payment option available - enrollment fee waived
- Re-enrollment after cancellation: $250 fee
- Controlled substances and benzodiazepines are not prescribed at this practice
- DPC membership is not health insurance
- HSA funds may be used for membership fees as of January 1, 2026

Yes! As of January 1, 2026, DPC memberships are HSA eligible. Most Flexible Spending Accounts can also be used. Check with your plan administrator for confirmation.
